Name Change (Minor Child) Package

The For The People Name Change (Minor Child) Package contains the standard forms needed to ask the court to legally change the name of a minor child or children. The Name Change (Minor Child) Package contains the following forms:

  • Petition for Change of Name for Minor Child
  • Notice of Petition for Change of Name for Minor Child
  • Affidavit of Petitioner
  • Decree for Order for Name Change of Minor
  • Affidavit of Mailing Notice of Application for Change of Name of Minor
  • Voluntary Appearance

Depending upon the county where the Petition is being filed there may also be local forms required by the Court; these are also included in the Package.

Name changes are sought for many reasons: perhaps your children are now part of a blended family and you would like everyone to have the same family name. Whatever the reason, name changes are common and usually fairly easy to carry out if both biological parents are consenting to the name change. If the child's biological mother and father are not the Petitioners (the people asking for the name change), then the parent who is not the Petitioner must be given notice ("served"). For The People cannot assist unless both of the child's parents can be made aware of the name change request.

Courts generally do not approve a petition for name change if the intent of the change is to defraud. After filing the Petition for Name Change, you will need to set a hearing date and then make arrangements for publishing the notice of name change in the newspaper; the costs of publication are not included in the amount paid to For The People .

Once the Court has granted your request for a name change and signed the Order for Change of Name of Minor you will need to obtain certified copies of the Order from the Clerk of the District Court. Once you have received a certified copy of the Order, you may use that certified copy to change the records with the Social Security Administration and other agencies.

Name Change (Adult) Package

The For The People Name Change (Adult) Package contains the standard forms needed to ask the court to legally change the name of an adult. The Name Change (Adult) Package contains the following forms:

  • Petition for Change of Name
  • Legal Notice of Petition for Change of Name
  • Affidavit of Petitioner
  • Decree of Name Change

Depending upon the county where the Petition is being filed there may also be local forms required by the Court; these are also included in the Package.

Name changes are sought for many reasons: perhaps you simply do not like your birth name and want a name that better suits you. Perhaps you are making or have made a career change, and feel that your birth name hinders you in your new job. Whatever the reason, name changes are common and usually fairly easy to carry out.   • Petition for Change of Names OR Petition for Change of Name and for Change of Name of Minor Child
  • Legal Notice of Petition OR Legal Notice of Petition for Change of Name and for Change of Name of Minor Child
  • Affidavit of Petitioner (one for each person whose name is being changed)
  • Affidavit of Mailing Notice of Application for Change of Name (if required)
  • Voluntary Appearance (if required)
  • Decree of Name Change (one for each person whose name is being changed).
